Customer Intake Form

This service is for the review of Retail HACCP plan for use in a licensed retail establishment. After completion of Association of Food and Drug Officials (AFDO) review, establishments will need to contact their regulatory authority for approval of HACCP plans and variances. To complete the first step in this process, you must complete the following intake form and:

Identify the type of HACCP plan you’ll be submitting

Provide contact information for person-in-charge and/or owner

Identify the establishment(s) where the HACCP plan will be used

Review and accept terms and conditions

After completing this intake form, you’ll receive an email with instructions on how to upload your sensitive documents to our secure FoodSHIELD platform.
