I've lost my Seafood HACCP Certificate...How do I get a replacement?
Prepare a letter to AFDO requesting a replacement certificate. The letter must include the following:
- Reason for replacement request
- Course information (as much as you can provide such as course#, date, location, etc.)
- Your full name and mailing address
- Enclose a personal check in the amount of $5.00 payable to "AFDO" for administrative services.
(Important Note: Payment must be in the form of a personal check, NOT a business check. Certificates are controlled documents and are the sole property of the individual to whom they are issued and cannot be purchased by a business - for the protection of the individual.)
