Course Documentation:
Upon approval of a course, the following documents are provide to the instructors:
- HACCP Instructor Worksheet Form (to be completed by course instructor) [View Xls]
- Student Information Sheet (to be completed by each student attending a registered course for copying) [View Doc]
- Course Evaluation Form (to be completed by each student) [View Doc]
- Invoice
At the conclusion of your course, please e-mail the following as an attachment to haccp@afdo.org, clearly referencing the HACCP course description and course number in the subject line:
- Completed HACCP Instructor Worksheet Form (Excel)
AND mail the following documents to AFDO at 2550 Kingston Road, Suite 311, York, PA 17402:
- Completed Student Information Forms (as detailed above)
- Completed Course Evaluation Forms (as detailed above)
- Course Payment (if applicable)*

NOTE: Internet students attending for the 3rd day only (face-to-face class) who have previously completed the Internet Segment One course must attach a copy of their admission ticket to their student information form in order to receive a certificate of HACCP completion. Please flag the student information form(s) for easy identification.
*Please submit the invoice to your designated Regional Affiliate Treasurer (when Affiliate is utilized). Include the number of student certificates to be issued. AFDO will issue certificates when payment is received.
